Before Passover, one should remove from their possession, all chametz (leaven products or food items not bearing a Kosher-for-Passover certification).
Chametz that your do not dispose of, should be sold to a gentile for the period of Passover.

It is preferable to complete and sign this form by hand. If you cannot do so, you may complete and sign a form online.
